All re-zoning applications must be complete when submitted to the town. Incomplete application will be returned to the applicant, and a meeting date will not be set until all materials are submitted. Planning & Zoning Commission meetings are on the second (2nd) Tuesday of each month at 6:30PM, in the courtroom at Town Hall.  Applications must be submitted to the town, at least 30 days prior to meeting.

Attention Contractors: Please print, complete and email the Town of Forest Park Building / Remodeling application to permits@forestparkok.gov.

The following information is also required along with the permit application. 

  • A complete set of project plans
  • Contractors' driver’s license (front and back)
  • Contractors’ License (front and back)
  • Liability Insurance (Town of Forest Park must be listed in the lower left-hand corner as the certificate holder)
  • Worker Compensation Insurance (If you do not have Workers Compensation Insurance, please complete the attached affidavit of exempt status form).
